This is displayed by selecting [New Form with Designs] in the [Form with Designs] category.
No. | Item | Brief description |

1 | Form list | Lists the forms that have been saved and can be selected. |
2 | Form information display | Displays information on the selected form (paper size, number of cells, cell size, and cell spacing). |
3 | Form preview | Displays the form selected from the list on the preview screen. |
4 | [Cancel] | Exits this screen and displays the Home screen. |
5 | [Create] | Displays the New form creation screen. |
